Individual Personal Plans, Weekly Planners and Reports 

Our first step in creating a results-driven framework of care for each young person is to construct an Initial Individual Personal Plan with the input of the young person’s Social Worker, their Pathway Plan, Lloydscare’ referral form, Lloydscare Risk Assessment and any primary observations made by the Casewaorker.

Lloydscare staff will work from this plan to prioritise key areas of support required, create initial weekly Individual Personal Plans, and inform monthly reports sent to the respective young person’s social worker.

Each young person’s case is reviewed on a weekly basis by the caseworker in consultation with our Operations Manager and/or qualified social care practitioner. The purpose of this case review is to ensure that actions tasked to Lloydscare are being delivered in accordance with the Individual Personal Plan, to ensure that all contacts with the young person are evidenced on files and the appropriate level of communication, information sharing and consultation with the local authority has taken place, and to manage any issues and risks.

Lloydscare will continue to assess the needs of the young person throughout their placement and these assessments will be reflected in the monthly reports to the respective Local Authority. This ongoing assessment process also enables us to continually identify, monitor and deliver the appropriate interventions and support needs. The assessment process is also a source of information in order to inform future planning and positive outcomes for the young people and to support the social workers in preparing documents such as the Needs Assessments, Pathway Plans, Housing Support Plans and, Move On Plans.

Lloydscare Reports

Our monthly reports sent to young people’s Social Workers aim to summarize all the work and interaction Lloydscare has undertaken with the Young Person, as well as Lloydscare’ ongoing Assessment and Future Plans.

Each Report addresses the following areas:

  • Health (Physical, Sexual and Emotional)
  • Psychotherapeutic Care – (Direct intervention, ongoing mental health assessment, effective links with mental health professionals)
  • Training/EmploymentProgress
  • Care and Hygiene (Cleanliness, Care and Presentation of Self and Home)
  • Identity (Racial, Cultural and Religious issues)
  • Social Relationships (Family, Friends, Partners and Support Network)
  • Personal Development (Independence Skills, Maturity and Decision making skills)
  • Finances (Budgeting and Personal Finance Management)
  • Lloydscare Support & Housing (Appropriateness and Effectiveness of Support Level and Accommodation)
  • Young Person’s Feedback on Support and Accommodation
  • Support Needs to be addressed in the following month
